Learning for Life registration form

Learning for Life is a partnership between The Smith Family and your family

Learning for Life provides

  • Support from a dedicated Family Partnership Coordinator. They can help with school attendance and transitions and connect you with support services in your community.
  • Access to educational programs for the whole family. These include assistance with literacy, numeracy or help from learning mentors.
  • Scholarship money twice a year to help with the costs of schooling.

Before you start

To join the Learning for Life program, please complete this registration form. The person with parental responsibility for the child or children should complete this form.

What you need to know

  • The form will take about 15 minutes to complete.
  • You cannot save the form, so please be ready to complete it at one time.
  • Your privacy is important to us. Our Data Collection Statement tells you about the personal information we collect, how we collect it and what we do with it.
